Maintenance Request Form Template

A maintenance request form is a structured submission that tenants or occupants use to report a property fault or request a repair. It captures the issue type, exact location, urgency level, and access availability so the property manager or facilities team can log, prioritize, and dispatch the work without needing additional back-and-forth contact.

The right moment is when repair requests start arriving through more than one channel — calls, texts, emails — and tracking them becomes unreliable. For multi-unit properties, introduce it at the start of each new tenancy so the process is established before a fault occurs. A recurring missed repair or a compliance audit is also a common trigger.

At minimum: unit number or location, issue category, a description of the problem, urgency level, and the tenant's preferred access windows. Adding a photo upload field for visible damage reduces contractor call-backs significantly. An optional field for whether pets are present in the unit is a small detail that prevents access complications on the day of the repair.

Use an urgency tier built into the form itself — emergency, urgent, and routine — so tenants self-classify at submission. Treat anything involving water, electrical faults, heating failure, or security as emergency regardless of the tenant's selection. Routing emergency submissions to a separate notification so they don't sit in a general queue is the most common gap in informal maintenance processes.

A maintenance request form is tenant-facing — it captures the reported problem and access details from the occupant's perspective. A work order form is the internal document generated after the request is reviewed, assigning the job to a specific contractor or technician with scope, timeline, and cost details. The maintenance request form feeds the work order; they serve different stages of the same repair process.
