A parent consent form is a document in which a legal guardian gives documented authorization for their child to participate in an organized activity, program, or service under another party's supervision. It establishes the guardian's approval, provides the organizer with emergency and medical information needed to keep the child safe, and creates a record that consent was given before participation began.

Use it at enrollment or registration — before a child attends their first session of any organized program, club, camp, or recurring activity. The key distinction from a field trip form is scope: a parent consent form covers an ongoing program relationship, so it should be collected once per enrollment period and updated when a child re-enrolls or when medical circumstances change.

At minimum: the guardian's full name and relationship to the child, two emergency contact numbers reachable during program hours, any medical conditions or allergies the supervising staff must know about, medication authorization if doses may be needed during sessions, and dietary restrictions for programs involving food. For programs with photography, add an image consent field so it is captured in the same pass rather than chased separately later.

A field trip permission form is event-specific — it covers one off-campus activity on a set date and typically includes transportation and destination details tied to that trip. A parent consent form covers enrollment in an ongoing program and authorizes participation across multiple sessions. Using a field trip form for program enrollment leaves gaps: it does not capture recurring medical needs, ongoing medication authorization, or program-wide photo consent.

Medical conditions, dietary needs, emergency contacts, and medication requirements can differ between siblings, and a shared form creates ambiguity about which details apply to which child.
