A student registration form is a structured intake form that collects the personal details, guardian or emergency contact information, and any medical or accessibility needs required to formally enroll a learner in a school, course, or educational program. Use it at the point of enrollment confirmation — when a prospective student or guardian has decided to join and needs to submit complete intake details before the program begins.
