Exit Survey Template

An exit survey is a self-completion questionnaire sent to employees who are leaving an organization, capturing their reasons for departure, satisfaction with their role, manager, and culture, and suggestions for improvement. It gives HR teams a scalable way to collect consistent offboarding feedback across all departures — producing aggregate data that identifies retention patterns rather than individual anecdotes.

Send it within 24 hours of a resignation being accepted, with a completion deadline set before the employee's last day. Employees are most candid during the notice period when the decision is final but professional relationships are still intact. Sending it after the last day risks non-response as the former employee moves on — completion rates drop significantly once access to company systems and the sense of professional obligation both end simultaneously.

An exit survey is designed for independent, self-directed completion — the employee fills it in on their own, which tends to produce more candid responses on sensitive topics like manager relationships or compensation dissatisfaction. An exit interview form structures a facilitated conversation between HR and the departing employee, which allows follow-up questions but may suppress honesty on uncomfortable topics. Exit surveys scale better; exit interviews go deeper. High-volume organizations often use both — the survey for all departures and a live interview for strategic or senior roles.

Use a defined list covering the most common drivers: compensation and benefits, limited growth or promotion opportunities, relationship with direct manager, workload or work-life balance, company culture or values misalignment, a better external opportunity, personal circumstances, and relocation. Keep the list to 8–10 options with a free-text 'other' field. Separating 'better opportunity elsewhere' from 'pushed factors' like manager relationship or culture is critical — conflating them masks whether turnover is driven by internal problems or external market pull.

For directional insight — identifying whether a pattern exists — 10–15 responses within a defined period or department is sufficient to flag a trend worth investigating. For statistically reliable segmentation by department, manager, or tenure band, aim for 30 or more responses per segment before drawing conclusions. Small organizations should aggregate data over 6–12 months rather than reviewing each departure in isolation, which produces noise rather than signal.
