Use it when engagement or referral rates start declining, before launching a premium tier, or when your subscriber base plateaus despite growth investments. Also run it after a major content shift to understand which changes resonated and which didn't.

This template includes 11 questions covering subscription history, content preferences, referral behavior, and upgrade interest. Keep it under 5 minutes to complete. If you want faster feedback, keep questions 1-7 and drop the premium tier questions; if you need deep insight into specific segments, add branching logic to show premium questions only to long-time subscribers.

Compare the Engaging Content Types field against the Recommendation Drivers field. Use cross-tabulation to see if subscribers who recommend you cite the same content formats as those they engage with most. This reveals whether people refer based on what they personally consume or what they think others should read.
